AM-001 G78 Oncology Records
GMGR states that oncology records must be kept for 50 years. Could you clarify if this applies to records which are only related to the actual speciality of oncology, or if they relate to all records where a patient has had cancer.
E.G. If a patient had skin cancer and the mole was excised and the patient kept under review this would be recorded as a dermatology record, and not oncology.
If the patient had a cancerous lesion which was removed and then the patient kept under review but no radiotherapy or chemotherapy involved this would be recorded under general surgery and not oncology.
Where a patient has had treatment for oncology and not purely surgery then the record should be retained as per G78. If there was no oncology treatment then the record should be retained as per the appropriate classification in GMGR.
In the examples given these patients never had any referral to or treatment in oncology so do not have any oncology notes and do not fall into the retention requirement for oncology.
AM-002 J10 CCTV Images
J10 states a retention of 28 days and 31 days, which is correct?  
Does this retention period apply to CCTV placed in family homes in relation to supervision of children with complex disabilities.
The ICO’s CCTV Code of Practice provides guidance to Data Controllers on the good practice standards to be adopted by those who operate CCTV.
There may well be instances where it is necessary to keep CCTV images for a longer period than the minimum period of 28 days (one month) stipulated in GMGR.  
As the Data Controller you should consult with your legal advisors if you require clarification on the retention of certain images. The retention period for J10 should read 28 days.
AM-003 D3 and G25
Crash Trolleys
During the review of GMGR a query about the retention of documents relating to crash trolleys was considered.  This appeared to be resolved in G25 of the new document but there is a second reference to crash trolleys in D3.  Can you help/clarify? The correct reference for the retention of records relating to Crash trolleys is D3.  Reference G25 should refer back to D3.
AM-004 K Human Tissue Authority Code of Practice 5
There is an error in the GMGR document. Reference is made to one of the Human Tissue Authority Codes of Practice, but the title is incorrect.
It is referenced (in section K, 'Pathology Records', pg 252) under the title of Code of Practice 5 "Removal Storage and Disposal of Human Organs and Tissue" however this was subsequently changed by the HTA (and slight amendment made to the Code itself) so the title is now simply "Disposal of Human Tissue".
The change of title has been confirmed and GMGR updated accordingly.
