Transforming Your Care

On these pages, we aim to provide up to date information on the process of transforming the Health and Social Care services in Northern Ireland.  We outline the context for change, and provide an overview of the proposals that were set out in the Transforming Your Care: Review of Health and Social Care in Northern Ireland (2011):, as well as links to relevant documentation and news.
The Health and Social Care Board (HSCB), in partnership with the Health and Social Care Trusts and other providers, is leading the implementation of many of the Transforming Your Care proposals. Following the publication of the Review, a draft Strategic Implementation Plan and accompanying draft local Population Plans were developed.  These draft Plans set out a proposed way forward for the implementation of TYC at regional and local levels and were initially published in July 2012 with revised draft Plans re-published as part of the public consultation process.
The public consultation was led by the HSCB and ran over a 14 week period from 9 October 2012 to 15 January 2013.
The consultation document – called Transforming Your Care: Vision to Action – set out key proposals for change across a range of service areas including primary care,  older people’s services, acute services and services for those with long term conditions. It explored how a focus on prevention, earlier interventions, integrated care and promotion of personalised care could enable more services to be provided in the community, closer to people’s homes where possible.  
Over 2,200 responses to the consultation were received from across Northern Ireland. Everyone’s voice in this consultation was important and many people took the opportunity to attend events or to participate in the consultation through social media.

In line with the Minister’s statement, work is now underway to take forward the specific actions that need to be taken to implement the proposals  and engage locally on proposals for specific service changes. Significant service changes will where appropriate be subject to further consultation focussed specifically on that change.
