Transforming Your Care

Welcome to the Transforming Your Care (TYC) website.  

On these pages, we aim to keep you up to date with the process of transforming the Health and Social Care services in Northern Ireland. We outline the context for change, and provide an overview of the recommendations that are to be implemented in health and social care services.
These pages also contain all the relevant documentation linked to the transformation process as well as providing you with all the latest news regionally and in your own area.

Consultation

The consultation – “Transforming Your Care: From Vision to Action” - was launched by the Minister for Health, Social Services and Public Safety on 9 October 2012.  The consultation outlines the Minister's proposals for change in Health and Social Care services in response to the Report “Transforming Your Care: A Review of Health and Social Care Northern Ireland” -  published in December 2011.  
The consultation document sets out key proposals for change across a range of service areas including mental health services, statutory residential homes, acute services and primary care.  It explores how a focus on prevention, earlier interventions, integrated care and promotion of personalised care could enable more services to be provided in the community, closer to people’s homes where possible.
The Health and Social Care Board (HSCB) is carrying out this consultation on behalf of the Minister. This means that, for the duration of the consultation period, the consultation document together with the Draft local Population Plans and Strategic Implementation Plan will be available on the HSCB's website at www.tycconsultation.hscni.netLink to an external website The Minister is keen to hear from any individual, or any group, who has a view on the proposals in the consultation material.  Your views are very important as the changes set out in the Transforming Your Care: From Vision to Action consultation document are substantial and explore how we might prepare our health and social care services for the future.
